Fill out and submit a separate form for each course you are adopting for. There are three separate fields per form in case you need to adopt more then one textbook per course (Textbook 1 adoption, Textbook 2 adoption and Textbook 3 adoption) if you don't need to adopt multiple books please leave the subsequent fields blank.
