If my application is incomplete or if information is missing from my file or has been falsified, I will not be admitted into the program. * Required
I鈥檓 a current degree-seeking student at 91涩漫 or I鈥檝e already applied to become an 91涩漫 student. * Required
I declared Surgical Technology as my academic program of interest when I applied to 91涩漫 or by working with an 91涩漫 advisor. * Required
I have reviewed and understand the pre-program course, GPA, and math requirements as outlined in the program guide. * Required
My program acceptance letter will be sent to the email provided in this application. * Required
I must be fully vaccinated/immunized prior to attending clinicals. I understand I run the risk of not meeting my surgical clinical case requirements due to limited site availability should I request an exemption. Vaccine exemptions are at the sole discretion of the clinical site and are NEVER guaranteed. Required vaccines include but are not limited to: TDAP, MMR, Varicella, Influenza, and COVID-19. * Required
As part of my clinical requirement, it may be necessary for me to travel up to 100 miles to my clinical site. Transportation is my responsibility and I will not be reimbursed for any travel-related costs. * Required
I will not register for required Surgical Technology courses until I have received official acceptance into the Surgical Technology program. * Required
